I have read and understand the Covid-19 Vaccination requirements of this event, I understand that I should always bring a mask with me to every event, I understand that any exemptions must be pre-approved prior to my attendance.

I understand that failing to provide proof of vaccination/approved exemption will mean that I cannot attend this event.
